Thank you for choosing our office for your dental needs. We strive to provide a comfortable environment where you'll feel confident in the level of care you receive. When you arrive for your first visit, please be prepared to complete our insurance and health information forms that will allow us to begin your dental treatment. For your convenience, our patient forms are available online. Feel free to print and fill out the forms and bring them to your first appointment. If your first visit is for a dental emergency, only the tooth in question will be addressed. Depending on the diagnosis, an additional appointment may be necessary.
